Cabell county currently has more than 500 food service establishments, which includes restaurants, bars, child care centers, hospitals, grocery stores, temporary food establishments and mobile food units. All food establishments are required to have a food service permit to operate. In order to obtain a permit the following information must be submitted to the health department for review:
Once plan approval is granted the establishment must be inspected and an operational permit issued prior to opening. Food service establishments are then inspected a minimum of once each six months in accordance with the U.S. Food and Drug Administration’s (FDA) 1999 Food Code which was adopted by the WV Legislature July 1, 2000.

Permits for mobile home parks are issued by the local and state health departments.
To apply for a permit, download and complete the application forms and submit them, along with a plan of your park to the local health department. Rules and design standards for the mobile home parks are found here on the web.

Plan Review Process for obtaining a Permit:
Need to contact the State Health Department and send plans to obtain a permit to modify, improve, contruct, or install a recreational water facility for public swimming or bathing.
If it’s a pre-existing facility. You need to contact your Local Health Department and send plans prior to opening, to schedule for an inspection of the recreational water facility to obtain a permit.
A Recreational Water Facility is defined as:
A body of water that is under the control of a person that has been modified, improved, constructed or installed, for the purpose of public swimming or bathing.
It includes, but is not limited to, bathing beaches; swimming, wading, and diving pools; water slides, spray pools, lazy rivers, and wave pools; spas, hot tubs, therapeutic pools, hydrotherapy pools, and whirlpools; facilities operated by communities, subdivisions, apartment complexes, condominiums, clubs, camps, schools, institutions, parks, mobile home parks, hotels, and similar recreational and public facilities.

Body Piercing and Tattoo Studios and Technicians are permitted through the local health departments and the State Health Department.

Request for Individual On-Site Water Supply and/or Sewage Dsiposal System Evaluation

Evaluations of Sewage and Water Systems for Property Transfers (Home Loan Evaluations)
Sanitarians inspect individual water supply systems and on-site sewage disposal systems to evaluate the systems for compliance with applicable laws and sanitary standards. The seller, the buyer, or the lender may make requests for evaluation. Please call Environmental Health at 523-6483, extension 262, to obtain Form SG-55. The top half is to be filled out with as much information as possible by the person making the request and returned to the Cabell-Huntington Health Department. A sanitarian will then contact the requester to schedule an inspection of the property. Listed below are links to the West Virginia Bureau of Public Health Office of Environmental Health Services web site for more information on these evaluations. There is no fee for services in Cabell County. The Office of Laboratory Services charges $15.00 per water sample for bacteriological analysis.

Contact your local health department to obtain a copy of the Division of Health’s rules for child care centers (64 CSR 21). In Cabell County, you can call the Cabell-Huntington Health Department at (304) 523-6483. 64 CSR 21 can be also be viewed online @ http://nrc.uchsc.edu/STATES/WV/wv_64.htm. Your local health department will also be able to provide you with food service (kitchen) requirements for day care centers. You will be required to contact your local health department to submit floor plans, to apply for a permit, and to request inspections.
