Environmental Health Services
Complaints Sanitarians perform routine inspections of Health Department-permitted facilities, such as restaurants, swimming pools, and day care centers, to monitor compliance with public health sanitation requirements. In addition to routine inspections, complaints from the public alert the CHHD to potential environmental health problems in the community. These complaints may be made against businesses, homeowners, individuals, and public property. The complaint is investigated to determine if it is justified as a public health issue requiring corrective action.
Complaints may be reported to our department in person, by mail, phone, fax, or email. The addresses and phone numbers are provided elsewhere on these web pages. Minimum information required is the address or directions and the nature of the public health complaint.

Water Supply Testing/Septic Tank Request for Individual On-Site Water Supply and/or Sewage Disposal System Evaluation SG-55 Evaluations of Sewage and Water Systems for Property Transfers (Home Loan Evaluations)
Sanitarians inspect individual water supply systems and on-site sewage disposal systems to evaluate the systems for compliance with applicable laws and sanitary standards. The seller, the buyer, or the lender may make requests for evaluation. Please call Environmental Health at (304) 523-6483, extension 262, to obtain Form SG-55. The top half is to be filled out with as much information as possible by the person making the request and returned to the Cabell-Huntington Health Department. A sanitarian will then contact the requester to schedule an inspection of the property. Listed below are links to the West Virginia Bureau of Public Health Office of Environmental Health Services web site for more information on these evaluations. There is no fee for services in Cabell County. The Office of Laboratory Services charges $15.00 per water sample for bacteriological analysis.

Water Wells A permit is required from the Health Department before drilling, modifying, or abandoning a water well. Please refer to the Office of Environmental Health Services web page for Individual Water Supply for useful information about wells and for the names of licensed well drillers.
Water TestingThe Office of Laboratory Services tests water samples that have been obtained by the county sanitarian. The homeowner may request drinking water tests for evidence of microbiological contamination and for specific chemical concentrations. Please click on the links below for more information. The Office of Laboratory Services does charge a fee for testing.
Tattoo and Body Piercing StudiosBody Piercing and Tattoo Studios and Technicians are permitted through the local health departments and the State Health Department. To apply for a permit, download and complete the application forms and submit them along with your plan to the county health department. Body Piercing Technician applications may be downloaded and submitted to the WV State Health Department. Recreational Water FacilitiesPlan Review Process for obtaining a Permit:
Need to contact the State Health Department and send plans to obtain a permit to modify, improve, construct, or install a recreational water facility for public swimming or bathing. If it’s a pre-existing facility. You need to contact your Local Health Department and send plans prior to opening, to schedule for an inspection of the recreational water facility to obtain a permit.
A Recreational Water Facility is defined as:
A body of water that is under the control of a person that has been modified, improved, constructed or installed, for the purpose of public swimming or bathing. It includes, but is not limited to, bathing beaches; swimming, wading, and diving pools; water slides, spray pools, lazy rivers, and wave pools; spas, hot tubs, therapeutic pools, hydrotherapy pools, and whirlpools; facilities operated by communities, subdivisions, apartment complexes, condominiums, clubs, camps, schools, institutions, parks, mobile home parks, hotels, and similar recreational and public facilities.

Mobile Home Parks Permits for mobile home parks are issued by the local and state health departments. To apply for a permit, download and complete the application forms and submit them, along with a plan of your park to the local health department. Restaurant Inspections Cabell County currently has more than 500 food service establishments, which include restaurants, bars, child care centers, hospitals, grocery stores, temporary food establishments and mobile food units. All food establishments are required to have a food service permit to operate. In order to obtain a permit the following information must be submitted to the health department for review:

A list of food service (kitchen) requirements for care centers can be obtained, also. Care centers must submit floor plans, apply for permits, and request inspections through the Health Department. They can also advise you on whom to contact for nutrition standards and menu reviews.
